Mr. Cigarran served as our Chairman of the Board from 1992 to July 2009. Mr. Cigarran served as our Chief Executive Officer from January 1993 until December 1997, and as our President from January 1993 to July 1996. From December 1997 to December 1999, Mr. Cigarran served as an advisor to us. Mr. Cigarran is a co-founder of Healthways, Inc. and served as Chairman of the Board of Healthways from 1988 until June 2011 and served as Chief Executive Officer of Healthways from 1988 until September 2003.
James A. Deal
Mr. Deal serves as President and Chief Executive Officer of Hospice Compassus, a provider of hospice care. From November 2001 to December 2005, Mr. Deal served as Chairman and Chief Executive Officer of INSPIRIS, Inspired Care for the Frail Elderly. From September 1998 to June 2001, Mr. Deal served as President, Chief Executive Officer and a director of Center for Diagnostic Imaging, Inc., a national network of outpatient diagnostic imaging centers. Mr. Deal served as Executive Vice President of Healthways, Inc., a publicly traded disease management company, from January 1991 to August 1998, and as President of Diabetes Treatment Centers of America, Inc. (now American Healthways Services, Inc.), a Healthways subsidiary, from 1985 to August 1998.

Henry D. Herr
Mr. Herr served as Executive Vice President of Finance and Administration and Chief Financial Officer of Healthways, Inc. from February 1986 to October 2001 and has been serving as a director of Healthways since 1988. Mr. Herr served as our Chief Financial Officer from April 1992 until September 1994 and as our Secretary from April 1992 until December 1997. From December 1997 to December 1999, Mr. Herr served as an advisor to us.

Joey A. Jacobs
Joey A. Jacobs, a director of AmSurg since June 2013, is Chairman and Chief Executive Officer of Acadia Healthcare, a rapidly growing publicly trade (NASDAQ: ACHC) behavioral health company. Before joining Acadia Mr. Jacobs co-founded Psychiatric Solutions, Inc. (PSI) (NASDAQ: PSYS) and served as Chairman, President and Chief Executive Officer April 1997 – November 2010.
Prior to founding PSI, Mr. Jacobs served for 21 years in various capacities with Hospital Corporation of America ("HCA," also formerly known as Columbia and Columbia/HCA), most recently as President of the Tennessee Division. Mr. Jacobs' background at HCA also includes serving as President of HCA's Central Group, Vice President of the Western Group, Assistant Vice President of the Central Group and Assistant Vice President of the Salt Lake City Division.
Mr. Jacobs currently serves as a Director for numerous healthcare industry organizations, including Cumberland Pharmaceuticals Inc., Mental Health Management and the Monroe Carell, Jr. Children’s Hospital at Vanderbilt. In addition, he is on the Board of the Nashville Health Care Council, which is an association of healthcare industry leaders working to promote the continued growth of Nashville as the nation’s healthcare capital, and he served as Chairman for the 2009-2011 years. Mr. Jacobs received the 2004 Southeast Entrepreneur of the Year Award presented by Ernst and Young and the 2007 Emerald Entrepreneurial and Excellence Award in the “Public Company Executive, Growth” category, awarded by Emerald Asset Management. Mr. Jacobs received his Bachelor of Science degree from Middle Tennessee State University.
Kevin P. Lavender

Mr. Lavender has served as Senior Vice President, Corporate Healthcare and Specialized Industries of Fifth Third Bank since December 2005. Prior to assuming that position, Mr. Lavender served as the Commissioner of the Tennessee Department of Financial Institutions from January 2003 to December 2005. In addition to his role as Commissioner, he served as the chairman of the National Regulatory Committee for the Conference of State Bank Supervisors and was a member of the Board of Directors. Prior to being named Commissioner, Mr. Lavender was co-founder and served as Executive Vice President of Administration and Banking for MediSphere Health Partners, Inc. from May 1996 to October 2002.
Cynthia S. Miller
Ms. Miller joined Univita, a provider of extended care and home care services, in early 2011. Previously, she was with WellPoint, Inc. and its predecessor, Anthem, Inc., which she joined in 1986. At WellPoint, Ms. Miller most recently served as Executive Vice President and Chief Actuary and previously as Senior Vice President and Chief of Staff to the CEO. At Anthem, Ms. Miller was Vice President and Chief Actuary.
John W. Popp, Jr., M.D.

John W. Popp, Jr., M.D. has served as a director of AmSurg since 2009. Dr. Popp, a board-certified internist and gastroenterologist, is Clinical Professor of Medicine at the University of South Carolina School of Medicine, which he joined in 1979. Since 1998, he has served on the Board of Trustees for the American College of Gastroenterology and is a past President of the College. Dr. Popp has been a Medical Director for Centocor, Inc. since July 2006, which is a unit of Johnson and Johnson that provides innovative biomedicines for debilitating immune disorders. Dr. Popp is a cum laude graduate of both Yale Medical School and Vanderbilt University.
